According to the World Health Organization, infertility is defined as the failure to achieve a pregnancy after 12 months or more of regular unprotected sexual intercourse. For women ≥35 years, evaluation is often recommended after six months.
Primary infertility (never conceived) and secondary infertility (infertility after at least one previous pregnancy or conception).
Causes can involve one or both partners. Common factors include hormonal imbalances, polycystic ovary syndrome (PCOS/PMOS), endometriosis, blocked fallopian tubes, reduced ovarian reserve, or male-factor issues such as low sperm count or motility. In roughly 10–15% of couples, thorough testing still finds no clear single cause, this is called unexplained infertility, and treatment options are still available.
Many causes of infertility can be treated, and when a specific cause is not identified, treatment options are still available based on the couple’s individual circumstances.
The Fertility Evaluation Process
A thorough, systematic evaluation forms the foundation of an effective treatment plan. A good assessment usually includes:
⦁ Detailed medical and reproductive history of both partners, menstrual patterns, previous pregnancies, losses or abortions, surgeries, medications, and family history.
⦁ Blood tests - Thyroid hormone levels and other relevant tests based on the patient’s history, ultrasound findings, and planned treatment or procedure. Ovarian reserve assessment, including AMH or other tests, is performed when clinically indicated
⦁ Semen analysis for the man, concentration, motility, morphology, and volume. Male factors contribute to roughly half of all infertility cases.
⦁ Pelvic ultrasound (USG) for the woman to examine the uterus and ovaries (looking for fibroids, cysts, polyps, or signs of PCOS/PMOS).
⦁ Tubal assessment when indicated (hysterosalpingography or similar) to check whether the fallopian tubes are open.
⦁ Discussion of lifestyle factors - weight, smoking, alcohol, stress

Treatment Options : Matched to Your Situation

Ovulation induction (OI) & programmed intercourse
Medication to help egg development in women with irregular or absent ovulation, combined with carefully timed (programmed) intercourse.
Intrauterine insemination (IUI)
Prepared (washed) sperm is gently placed into the uterus around the time of ovulation using a thin catheter. It is often used for mild male-factor infertility, unexplained infertility, or selected cervical factors.
In vitro fertilisation (IVF) / ICSI
Eggs are collected and fertilised with sperm in the laboratory using standard IVF or ICSI when indicated. The resulting embryo is then transferred to the uterus. IVF may be recommended when clinically indicated based on the diagnosis and individual circumstances.

Minimally invasive procedures to treat endometriosis, remove fibroids or polyps, or address selected tubal disease when these are contributing to infertility.
